The diagnosis of superficial femoral artery pseudoaneurysm as a complication after penetrating wound trauma is rare. Its late presentation is really exceptional and very difficult to diagnose unless there is a very high suspicion. We present the case of a 17-year-old male, who presented with pain and increased volume in the left thigh for 10 days, after having suffered a sharp injury trauma, with two wire barbs, two months ago, on physical examination presented two 1 cm scars. Ultrasound was performed with linear transducer, without showing any evidence of deep lesion. Due to doubt and strong suspicion, ultrasound was performed with convex color Doppler transducer, which showed characteristic flow in "ying - yang" compatible with pseudoaneurysm, which was confirmed unequivocally with angiotomography compatible with single saccular dilatation of the superficial femoral artery, which was successfully resected.
